Our services are provided to clients who need and use them (further as „clients“) because of their limited self-sufficiency caused by age, chronic illness or health disablement and who need the help of other person.
Our services are provided in natural social environment of the clients, cover the activities that need to be secured and are specifically targeted to survivors of the Holocaust, victims of the former national socialist brutality.
1. Basic consultancy targeted on social matters
In the framework of such consultancy necessary information are provided to solve adverse social situation. This service includes basic consultancy, mediation of contact with social life, therapeutical activities and help with asserting rights and interest.
2. Social services – personal assistance
These services include help with daily self-care activities, help with personal hygiene, help with functioning of ones´ home, mediation of contact with social life and help with asserting rights and interest.
Our services might be asked for by the clients or his/her relatives or friends. The commencement or termination of social care is planned together with the client, his/her relatives or social workers of KDP EZRA.
The process of commencement of social care services
The social worker visits the services´ applicant in his/her home or institution where he/she resides (hospital, care institution etc). Clients´ needs are professionally assessed together with the client and both agree on the optimal range of the services. The social worker then prepares the agreement of the services´ rendition by KDP EZRA or mediates the services´ rendition by other institution.
3. Home care
Home health care is provided along the recommendation by the attending physician if the capacity of KDP EZRA allows it. The physician might be a practical physician or a hospital physician. The individual care plan is commenced and terminated by matron together with the client according to the requirements of the attending physician.
The process of commencement of home health care
The attending physician or hospital physician recommends and prescribes the 06 voucher, where content and range of needed health care is described. After the delivery of such voucher to KDP EZRA, the matron or nurse visit the client and together they agree on exact care plan.
4. Ergo therapy
The ergo therapy services are provided along the recommendation by the attending physician or KDP EZRA workers – matron, social worker or ergo therapist.
The process of commencement of ergo therapy services
The ergo therapist visits the client in his/her home or institution where he/she resides (hospital, care institution etc). Based on the entrance examination the ergo therapist designs the ergo therapy plan (individual therapy, consultancy, ergo therapy utilities etc.)
5. Ergo therapy utilities rental service
The utilities are rented along the recommendation of the KDP EZRA ergo therapist. The purpose of the rental service is the possibility to choose and try out the most suitable utilities. Information about rental service is provided by the KDP EZRA ergo therapist.
